 At Live 2 Give Home Health Care, we provide compassionate, personalized care through Missouri’s Medicaid-funded Consumer Directed Services (CDS) program. If you have Medicaid, you may qualify to choose a trusted family member or friend to care for you—and they can get paid. 


To qualify for Consumer Directed Services, you must:


  • Be eligible for Medicaid
     
  • Need assistance with daily living activities
     
  • Be able to direct your own care (or have someone you trust to help manage it)

A Senior Social Services Specialist from the State of Missouri will visit your home to assess your eligibility for the Consumer Directed Services (CDS) program. During the visit, they will also determine the number of care hours you may receive each week.


2. Choose a Caregiver 

 You choose a caregiver you trust—your daughter, son, a relative, or a close friend. We’ll then schedule a brief orientation call to complete your enrollment and get you started.

Common Questions About Missouri's Consumer Directed Program (CDS)

 To qualify, you must be 18 or older, have a physical disability, be on active Medicaid, meet the level of care required for a nursing facility, and be able to self-direct your care. 

Live 2 Give Home Health Care services are designed to benefit seniors and individuals with disabilities or chronic health conditions who require assistance with daily living activities.

 Self-directing means you can hire, train, supervise, and if necessary, dismiss your caregiver. You must also approve timesheets, monitor visits, and report changes to your care needs. 

 No. Missouri law does not allow spouses or legal guardians to be paid caregivers under CDS. 

Yes, caregivers can live with you, but they cannot be your spouse or legal guardian. 

Caregivers can help with bathing, grooming, dressing, toileting, meal preparation, medication reminders, laundry, cleaning, and essential transportation. 

 Hours are recorded using Electronic Visit Verification (EVV), which tracks clock-in and clock-out times. 

 Yes. CDS can cover up to 60% of the Medicaid cost maximum, though exceptions may be reviewed if your needs are greater. 

 Yes, essential transportation such as work, school, and errands is included as long as it is part of your care plan. 

 Yes, if those appointments are not already covered under Medicaid’s Non-Emergency Medical Transportation (NEMT) program.
